First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Then a surgery appointment will be scheduled. Occasionally, surgery can be done the same day as the consultation but this is uncommon.

Please assist us by providing the following information at the time of your consultation:

  • – Your surgical referral slip and any X-rays if applicable
  • – A list of medications you are presently taking
  • – A list of drug allergies

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit and surgery appointments.

A pre-operative consultation and examination is mandatory for patients undergoing IV anesthesia for surgery. Prior to your surgery appointment, please do NOT eat or drink for 8 hours before your appointment. You will also need a responsible individual to drive you home.

We have complete digital radiographic capability. Your dentist or physician can forward the images to us electronically or have them mailed. It is common that additional radiographs will be required at the initial visit.

Boise OMS is open Monday - Friday from 8 am until 5 pm. We prefer to schedule surgeries in the morning and consultations in the afternoon and we will schedule your appointment as promptly as possible. We try our best to stay on schedule to minimize your waiting but due to the fact that Boise OMS provides surgical services and treats emergency cases unexpected delays can occur. We appreciate your understanding and patience.

For your convenience we accept Visa, MasterCard, Discover and Care Credit. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered. If you have questions regarding your account, please contact us.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement and most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

Surgical Information

Before Surgery (Intravenous Anesthesia)


  • – Do not eat or drink (including water) for eight (8) hours prior to the surgery appointment.
  • – Do not smoke before and after surgery.
  • – A responsible adult must accompany the patient to the office, remain in the office during the procedure, and drive the patient home.
  • – Do not drive a vehicle or operate any machinery for 24 hours.
  • – Wear loose fitting clothing with sleeves which can be rolled up past the elbow.
  • – Remove all contact lenses, jewelry, and dentures prior to surgery.
  • – Do not wear lipstick, excessive makeup, or nail polish on the day of surgery.
  • – Inform the office of any illness such as a cold, sore throat, stomach or bowel upset.
  • – Please inform the office of all routine oral medications.
